Summit Youth & Family Services Meetings

​In 2014, Meridian Institute was asked to facilitate a series of meetings of the Juvenile Intervention and Prevention Partnership in Summit County, Colorado. The Partnership is a small group composed of key organizations that provide services for youth and their families in Summit County. The group focuses on prevention and intervention activities, including youth and family activities, voluntary prevention programs, health and mental health services, and an adjudicated program.

Meridian facilitated the development of a draft “system map” to identify key support services available to youth and their families. Participants discussed how individuals and families move through the system, identifying major gaps in services or missed connections between services. Subsequent meetings have focused on strategic action planning to address these gaps.
